2) Review and sign Telemedicine informed consent form by clicking here


3) If not already done, Schedule an appointment for Telemedicine by calling office at 972 478 8800

​

4) Office staff will let you know the amount of Copay/Coinsurance/Deductible for your Telemedicine virtual visit after reviewing your insurance. It can be paid online using the "Pay Now" button below.

** If you do not have insurance and paying out of pocket, office staff will let you the final amount which can vary on case by case based on complexity.


5) Our Doctor will call you at the time of the scheduled Telemedicine visit with Meeting ID Code. Open the RingCentral Meeting App and add this code into "Join a Meeting" field.
